1Your information

2Core build

How the app rolls out across your ten states.

Everything needed for a working first version. Uncheck anything you do not want.

3Add ons

Optional. Check anything you want included now. Add ons can also be added later at the same price.

4Monthly plan

Keeps the app running, backed up, and updated after launch. Billing starts the month after launch.

Outside services Elevare pays for

The app needs a few outside services to run. These are not included in the prices above. Elevare opens them in its own name and pays the providers directly, so Elevare owns every account. Each item above lists exactly what it needs.

The biggest decision is privacy level. With standard hosting, outside costs are usually $40 to $150 a month. If your compliance lead requires every service holding referral information to sign a privacy agreement (BAA), hosting alone starts around $599 a month, and video assessments add about $500 a month. Healing Tek will price both paths for your compliance lead before anything is bought.

  • Domain name and business email
  • Secure hosting and database, with a signed privacy agreement if your compliance lead requires one
  • Text message alerts, billed per message
  • Video call minutes, if video assessments are chosen
  • Apple Developer account ($99 a year) and Google Play account ($25 once), if App Store apps are chosen

If Healing Tek pays any of these on Elevare's behalf, they are billed back at cost plus a 15% handling fee.

4bRequests beyond this list

Anything you request that is not listed here gets a written estimate within 2 business days, before any work starts. Every estimate shows:

  • Price and time: the build cost and how many weeks it adds
  • What it takes: the work involved, in plain language
  • What you would buy: any outside service, license, or account, with its current price
  • Who builds it: Healing Tek, or an outside specialist when the work needs one

Some work needs a specialist beyond Healing Tek, and we will always say so up front. Examples: connecting to medical records or billing systems that only certified partners can access, a formal security or HIPAA risk assessment, legal review of privacy documents, and professional translation. Healing Tek can coordinate the specialist; their fees are quoted by them and paid by Elevare.

5What Elevare provides

The timeline starts once these are received.

  • Every home: name, address, ZIP code, total beds, open beds, populations served, weekly rate, house rules, intake contact, and 3 to 6 photos
  • Outside housing partners Elevare refers to, with the same details if available
  • Which team receives referrals for each state, with the email and phone for alerts
  • Staff who need logins and their roles
  • Logo files, brand colors, and website text or approval of drafted text
  • A decision on the privacy and hosting level after reviewing it with your compliance lead
  • The outside service accounts listed in section 4, opened in Elevare's name, or written approval for Healing Tek to buy them at cost plus 15%
  • If App Store apps are chosen: a free D-U-N-S number for Elevare's legal entity, requested from Dun & Bradstreet through Apple's lookup tool right after signing. Apple requires it for organization accounts. It takes up to 5 business days to arrive, plus up to 2 more before Apple can use it. Use the exact legal name on Elevare's state registration, not a trade name, and have registration documents ready in case Dun & Bradstreet asks

6Notes and questions

7Agreement

Parties

This agreement is between Healing Tek LLC, Indianapolis, Indiana ("Healing Tek"), and the organization named in section 1 ("Client"). It covers the items checked on this form and nothing else.

1. Scope

Healing Tek will design and build the checked items. Work not listed here is a change request and is quoted separately in writing before it starts.

2. Price and payment

The one time total shown on this form is the full build price. Client pays a 50% deposit to start. The remaining 50% is due before launch. The app and website go live after final payment. Payment by Zelle, Cash App, card or online invoice, or check payable to Samuel Doolittle.

3. Monthly plan

If a monthly plan is chosen, it is billed monthly starting the month after launch and can be canceled with 30 days written notice.

3a. Outside services and costs

Anything needed from outside providers to run the system officially, including the domain, email, hosting, database, privacy agreements, text messages, video minutes, app store accounts, and any paid software or licenses, is Client's responsibility and is not included in the build price. Client opens these accounts in its own name and pays the providers directly. If Client asks Healing Tek to pay any of these costs, Healing Tek bills them back at cost plus a 15% handling fee, with receipts, due within 15 days of invoice. Healing Tek may pause work or service while such an invoice is past due.

4. Timeline

The estimated timeline shown on this form begins when the deposit and the items in section 5 are received. If Client does not respond for 30 days, the project pauses and the timeline moves by the length of the pause.

5. Changes

Two rounds of changes are included. Extra rounds are $75 each.

6. Ownership

Client owns its domain, content, data, photos, and service accounts. Healing Tek keeps the right to reuse general code and design methods that contain no Client data or branding, and may show the work in its portfolio with sample data only, unless Client opts out in writing.

7. Privacy and client data

Referrals may include health information. Before building, both parties will agree in writing on what information is collected, who can see it, and where it is stored. Healing Tek will use services that offer signed privacy agreements where Client requires them. Healing Tek does not certify that the system is HIPAA compliant; Client's compliance lead reviews and approves the setup before launch. Healing Tek will not sell or share Client data.

8. Cancellation

Either party may cancel in writing. If Client cancels after work begins, the deposit is not refunded. Work completed is delivered to Client on cancellation.

9. Limits

Healing Tek is not responsible for placement decisions, bed accuracy entered by Client or partners, or outages of outside services. Total liability is limited to the amount Client paid under this agreement.

10. Signatures

This agreement takes effect when both parties sign. Typed names below count as signatures.
